How This Works

DUI Assessment Specialist is a unique service providing alcohol and drug evaluations via the telephone and Internet. We offer a more convenient and confidential method of assessing your alcohol or other drug use. Although many people seek evaluations following driving offenses, DUI Assessment Specialist provides evaluations specifically for DUI Offenders. DUI Assessment Specialist has licensed and Certified Addiction Counselors to complete your evaluation.

DUI Assessment Options

You many choose one of the following options for completing your evaluation:
1. Toll free Telephone Evaluation: Call us toll free at 1-855-233-7100.
2. Same Day Evaluation (RUSH EVAL): Can be offered if contacted by no later than 10a.m. Emergency DUI Assessment requests will be completed, faxed and/or emailed to you or your designee that day by 5:00p.m. However, an additional fee of $50.00 will be charged for such service.
3. Hours of operation are Monday – Friday 8a.m. – 5p.m.

2. We will schedule your appointment time with a certified counselor to be completed by telephone or the Internet.
3. If you choose the Internet option, we will provide you with a webcam within 48 hours.
4. Payment in full is required at the time of scheduling.*

*Refund Policy: To qualify for a refund due to disallowance by the court of record, your probation officer, or a state motor vehicle employee, you must apply for this refund with 30 days of your initial payment and email the reasons the evaluation was disallowed to us at refund@duiassessmentspecialist.com or fax to (513) 737-0026.  If you fail to keep your scheduled appointment and do not reschedule, a $75.00 no show fee will be charged.



FAQs

How long does an assessment take?

Each Assessment takes about 50 to 60 minutes with no waiting lists for appointments. Services are accessible by anyone able to use or access a computer or telephone.

Are my records kept confidential?

Yes. Under Federal Confidentiality rules (42CFR, Part 2), we are not allowed to disclose any information about you to any party without your expressed consent; but you may provide your report to anyone you determine.

Will I be able to get copies of my evaluation later?

Yes. The original signed Evaluation Report and Referral Reports will be sent directly to you from your counselor via email, fax or direct mail within three (3) working days after your counselor has spoken with you. You will also receive a copy of your evaluation online. It is your responsibility to make copies and forward the original report to other entities as you are required.


I have special forms, what do I do with these?

If you have forms that need to be filled out or signed by one of our counselors or if you have a referral letter stating what is needed from you Assessment Counselor, please fax a copy of these forms to (513) 737-0026 prior to completing the assessment.

Can you refer me to a DUI provider or treatment center?

If you require substance abuse assessment, or intensive outpatient or inpatient treatment at a treatment center, we can refer you to a Treatment Provider that meets your needs. In many cases, this can also be done online.
